Home Again Program Evaluation
Rural Alaska Community Action Program, Inc. (RurAL CAP) and Anchorage Community Mental Health Services (ACMHS) have received SAMHSA funding to collaboratively implement the Home Again program, which provides individuals who experience chronic homelessness and co-occurring substance abuse and/or mental health disorders with safe and secure housing and concurrent intensive case management. CBHRS has contracted with RurAL CAP and ACMHS to conduct a program evaluation of the Home Again program. Outcome data will be analyzed to examine health, family, living conditions, criminal justice involvement, etc. at intake, 6 month follow-up, and discharge. A process evaluation utilizing qualitative interviews and focus groups is also being conducted in order to assess program effectiveness and barriers to service implementation.
