Mini-Grant Application

You’ve completed the Mini-Grant Application when you have: 

q       Filled out the Cover Sheet contact information

q       A Tribal Official has signed and dated the Cover Sheet

q       Described a concern on the Concern Statement

q       Identified possible causes of this concern on the Causes Statement

q       Identified the question you want to address or other action your community wants to take in the Mini-Grant on the Question or Other Action Statement

q       Developed a Proposed Budget

q       Completed and have signatures on the Community Commitment of People and Resources

When you have completed the mini-grant, make a copy and mail the original to:
